COVID-19 Vaccine Updated Information
From April 6th Onwards clients who are 18+ are now eligible to get vaccine from Crofton Pharmacy (MODERNA VACCINE IS ONLY AUTHORIZED TO BE USED FOR 18 YEARS AND OLDER CLIENTS - as per CDC guidelines).
This form will allow you to make 1st & 2nd Dose appointments at the same time. If younger than 18 years, PLEASE DO NOT SCHEDULE AN APPOINTMENT. Form will not allow submission if you do not fall under the eligible category. If you are not able to make an appointment or the time slots are greyed out that means no open time slots are available. Pharmacy cannot schedule appointment over phone. Please refrain from calling the Pharmacy for appointment. Also please don't schedule multiple appointments, if you would like to cancel an appointment use "CANCEL APPOINTMENTS" tab. AGE & IDENTITY PROOF WILL BE REQUIRED DURING YOUR VACCINE APPOINTMENT. MEDICARE PART A & B card required for 65 and older clients. If you have active insurance, please bring Prescription card for vaccination (Medical insurance cards are not accepted by the Pharmacy). If you are 65+ and are not able to make appointments online, please call the Pharmacy and we will allow walk-ins based on vaccine availability for that day. (Not applicable to any other age group). Please wear short sleeve shirts to your appointment. Second Dose Appointment should be booked at least 28 days apart from the 1st Dose Appointment. Please select the earliest possible 2nd Dose Appointment so that we can help other clients like you who are looking for COVID-19 Vaccine. Thanks for your patience.
Clients who have already received the 1st dose vaccine from Crofton Pharmacy up till 3/27 are now requested to book their appointments from our website by clicking "Book Second Dose Appointment" button.
Please make sure you book second dose appointment 28 days apart from your 1st dose appointment. If you book an appointment which is not 28 days apart, pharmacy will reschedule that appointment. If you are not able to make an appointment or the time slots are greyed out that means no open time slots are available.
If you are close to 42 days due on your second appointment and are not able to find appointments online, we request you to call Pharmacy.

On the day of your second appointment please bring your vaccination card which shows the 1st appointment date. Please wear short sleeve shirts to your appointment.
Clients who wants to cancel their already existing appointment should use this feature. Please refrain from using this feature if you do not already have an existing appointment.
If you are not falling under the Phase 1a and Phase 1b eligibility criteria for the COVID-19 Vaccination, please express your interest by clicking the "COVID-19 VACCINE WISHLIST" and entering your details.
